How to make a referral

Referrals are made through any professional working in the health, social services and voluntary sector. We also welcome applicants form carers and relatives.

To make a referral the form titled "Application for Day Centre Attendance" should be completed, which you can request here, and sent to:

One of the members of the assessment team will then make a home visit to decide on the suitability of the referred person for attendance to the centre.

Visitors are always welcome, but please ring our office before you come on 0208 802 5901.

Also we welcome enquiries from people wishing to offer their services as helpers either working in the centre or as drivers, or both.

Training is provided and all out of pocket expenses are paid.

Finally, carers and relatives may wish to spend a day with us, either as helpers or see the sorts of activities that we do, just let us know and we will always be pleased to see you.
